Instructions for Downloading and Printing GIM PA Form


Download GIM PA Form

  1. Click Download GIM PA form button to download the GIM PA form
  2. In the Save as dialog select Save to desktop
  3. Open and Print the GIM PA Form (default file name: GIM_2013_Admission_form.pdf)

Note: You will need Acrobat Reader to View and Print GIM PA Form
